Lifesaving Information For Emergencies
The FILE OF LIFE enables emergency first responders to obtain
critical information quickly when an individual is unable to. The
FILE OF LIFE is a card with your medical history that includes your
emergency medical contact, insurance policy, social security number,
health problems, medications, dosages, allergies, recent surgery,
religion and a health care proxy. The card is kept in a red plastic
pocket and is held with a magnet to the outside of the refrigerator.
The FILE OF LIFE is available at the following Des Plaines
locations:
- The Civic Center, Health and Human Services Division, Room 502
- The Des Plaines Public Library, 1501 Ellinwood
- The Police Neighborhood Resource Center, 1928 E. Touhy
- The Des Plaines Community and Frisbie Senior Centers,
- 515 E. Thacker Street and 52 E. Northwest Highway
