File of Life

Lifesaving Information For Emergencies

The FILE OF LIFE enables emergency first responders to obtain critical information quickly when an individual is unable to. The FILE OF LIFE is a card with your medical history that includes your emergency medical contact, insurance policy, social security number, health problems, medications, dosages, allergies, recent surgery, religion and a health care proxy. The card is kept in a red plastic pocket and is held with a magnet to the outside of the refrigerator.

The FILE OF LIFE is available at the following Des Plaines locations:

  • The Civic Center, Health and Human Services Division, Room 502
  • The Des Plaines Public Library, 1501 Ellinwood
  • The Des Plaines Community and Frisbie Senior Centers,
  • 515 E. Thacker Street and 52 E. Northwest Highway
